iPhone & iPad (Safari Only)

Open the guide link in Safari, tap the Share icon (square with upward arrow) at the bottom of the screen, then select "Add to Home Screen" and tap "Add." Note: this won't work from Chrome or an in-app browser like Gmail.

Android (Use Chrome)

Open the guide link in Chrome. A prompt will appear asking you to "Add to Home Screen." Tap "Add," confirm on the pop-up, and within about 10 seconds the shortcut appears on your home screen.

Troubleshooting
